What is a Proxy?
For purposes of the CAMC Patient Portal, a proxy is an individual who is not the patient who has been given permission to access the patient’s health records on the CAMC Patient Portal.
Proxy access for adult patients - age 18 or older
An adult patient or an incapacitated adult patient’s legal representative may grant proxy access to any other adult upon completing the appropriate authorization form, as described below.
An adult patient may revoke proxy access at any time by calling 877-621-8014.
Proxy access for pediatric patients - birth through age 11
A parent or representative with appropriate legal documentation may have access to their pediatric patient's health record on the CAMC Patient Portal as a Proxy until the child reaches the age of 12; the Proxy Access Authorization form must be completed. Upon the attainment of age12, the Proxy's access will automatically be revoked, as described below. Please be advised that applicable state and federal law contains privacy protections that may impact foster parents’ and adoptive parents’ ability to access a minor’s medical records through the CAMC Patient Portal.
Proxy access for adolescent patients - ages 12 through 17
Note to Parents: For parents and guardians it may be hard to accept that you may be unable to review your all your child’s medical records after they reach a certain age. Federal and state guidelines and CAMC policies are in place to protect adolescents that need to keep some healthcare private. Due to the current technical infeasibility of separating certain sensitive medical records from the CAMC Patient Portal, a parent or legal representative will not have automatic proxy access to their adolescent patient's health records on the CAMC Patient Portal once the child reaches age 12.
The adolescent patient may be enrolled, at age 13, to have direct access to their patient portal, unless restricted by the adolescent's provider. The parent or legal representative may still obtain a paper copy of most of the adolescent patient's health records in the Health Information Management Department by signing the appropriate release of information authorization. However, when a parent requests paper medical records for an adolescent patient, the same state and federal protections will apply.
An adolescent may reinstate the proxy access of a parent or representative with appropriate legal documentation, as set forth below. An adolescent patient may also revoke reinstated proxy access at any time by calling 877-621-8014.
Steps for requesting a proxy account
1. Fill out all fields on the appropriate form.
2. Submit the form to CAMC with a copy of your photo ID (or actual ID if submitting in person) to:

- Once your form has been submitted and the proxy relationship approved, you should receive an invitation via email within 72 business hours to claim the account on behalf of the patient.
- Adult and adolescent patients may remove proxy access by calling 877-621-8014 and providing name and date of birth.
