Sherry Sheets is an LPN navigator.
She spoke with a congestive heart failure clinic patient on a Friday afternoon and adjusted their medications.
The patient was scheduled for an IV Lasix treatment in the CHF clinic on Monday.
Sheets reached out to the patient on Saturday and again on Sunday to verify that her symptoms were improving.
The patient then called Sheets late Sunday night to tell her that she did not have a ride to her appointment on Monday for her lasix treatment.
Instead of cancelling the appointment, Sheets picked up the patient and brought her to the appointment in the CHF Clinic.
The patient ended up needing hospital admission for further treatment.
Sheets then called all of the other physician offices to cancel the patient’s appointments.
